do i have to do the five categories to get the petals, can you explain.? Sometimes i get the petals others don't

I f I understand it correctly, this feature is ment to reward active players, If you are an active player you'll get petals randomly, sometimes an action will be rewarded, sometimes not, sometimes with a single point other times with more.

Best way I've found to get petals so far is doing the amateur movie virgin defloration, seem to get one everytime I start one, and probably about 1 in 20 gives 100.  Other than that random i think, though I have never gotten one from a Pro movie.

Doing any one of the five categories will give you pendents. Though some have more chances of gaining pendents than others. You should also note that you might not get pendents every single time. Its a random chance :)
